Re: Ms. Bethany Tailor
I am writing to update you on the progress of Ms. Tailor, a patient of yours who was recently self-admitted to Mary Public Hospital her self due to decompensated schizophrenia. She has responded well to the treatment during her hospitalization, and is now she is ready to be discharged back into your care.
Ms. Tailor was admitted on 01/03/2018, after she was experiencing auditory command hallucinations telling her to harm herself, along with visual hallucinationhallucinations and delusion. She was agitated and aggressive. Therefore, risperidoneRisperidone was commenced.

On 10/03/2018, Ms. Tailor showed significant improvement regardingof her symptoms, and she was able to minimize delusions and focus on activities of daily living. Her nursing management was aimed to at assessing her for objective signs of psychosis and redirecting her from delusions. Additionally, it helped her to comply with her medication, and to maintain behavioral Ccontrolcontrol.
OnIn today’s assessment, it was found that Ms. Tailor She is recovering well. She has a good insight and fair judgment; however. However, she could decompensate if she diddoes not Ccomplycomply with her medications or abuseif she abuses substances. She has been discharged on risperidoneRisperidone tabs 4 g atat 4g per night, and she can take addition risperidone 1 mgadditional Risperidone at 1mg twice daily if she has agitation or psychosis.
