Dear Anna,
I am writing to refer the above-mentioned patient, who was presented to us with an enlarged spleen and high blood pressure. He undergoneunderwent surgery and is now inmaking remarkable progress. He is for transferto be transferred into your care to facilitate continuous nursing management and recovery.
On theApril 4th of April 2013, Mr. Emily was admitted to the hospital due to somea viral infection which resulted toin the enlargement of his spleen. He has had a history of high blood pressure years back that reoccurin the past which occurred again prior to his hospitalization. He was afebrile with no signs of bleeding, and his blood pressure was normal upon discharge.
Moreover, following the surgery procedure, Mr. Emily is now required to take a 100 mg Aspirin tablet 100mg. Several vaccine injections like Pneumococcal, meningococcalMeningococcal, and haemophillus influenza type bHaemophillus Influenza Type B are required as well. Apart from the medicationmedications that he needs to take, it is also significantimportant to regularly change the dressing in his surgery site.
In addition, Mr. Emily is advised to avoid travelling and doing extrenousextraneous activities. If necessary, he needs not to be in contact with water. He should be well monitored for his Penicillin injection and a full blood exam needs to be followed up in our institution.
In view of the aforementioned, kindly provide a continuous nursing care and medical management to Mr. Emily as deemed necessary to ensure that a full recovery is achieveachieved. I have attached the needednecessary documents for transfer.
