On 5-11-17, I was contracted with Aflac and was excited to get
started. Unfortunately, I wasn't able to start at the time duefor to
medical reasons.

In the later part of 2009, I sustained a head injury that caused
several skull fractures and an acute subdural hematoma. Due to this
injury, I havesuffer from chronic pain that causes me to struggle with bouts of
depression that can last a week or months. There is no predictable
timepredictable timing.

When I was able to go back to work, my contract with Aflac was
terminated, due my insurance license laspseinghaving lapsed. I forgot to pay the 2
year2-year license fee. This has all been corrected.
